The concerned person must come and apply with his/ her pledge.

Application form with the filled genealogical data.

Application form to issue the copy of the national identification card for the lost / destroyed one (including with the recommendation of living from the quarter/ village administer and recommendation of actual lost from the police station) with the Form of Htwe-4.

Household residence registration certificate including the name of the concerned person. (origin and copy)

Parents’ National Identification Cards (origin and copy)

If the card is destroyed, the number of the destroyed card shall be submitted.

4 Photos (0.8 and 0.8 inch) (no glassware)
