Applying the national identification card on the age of 10 or the age of 18

For the age of 10, the concerned person and the person who is the head of the family must come and apply. For the age of 18, the concerned person must come and apply himself/ herself.

Household residence registration certificate including the name of the concerned person. (origin and copy)

Application form with the filled genealogical data.

For the age of the concerned person, recommendation from school or Certificate of Birth (origin and copy)

Parents’ National Identification Cards (origin and copy)

Certificate of blood type of the concerned person (origin)

Recommendation of living from the quarter/ village administer
-
4 photos (0.8 and 0.8 inch) (no glassware)
Exchanging the national identification card from the age of 10 to the age of 18

The concerned person must come and apply himself / herself.
